Register with our dental practice in Welling, Kent – Bellegrove Dental Surgery.
Switching to a new dental practice is a quick and easy process. You would need to make you an initial appointment to see your dentist. You will be required to fill out a medical history questionnaire detailing past and present medical conditions and any drugs you may be taking.
This questionnaire can either be downloaded from the link below, or alternatively can be filled out at reception when you arrive for your first appointment. If you wish to fill your medical form out at the surgery, please arrive 10 minutes before your appointment time to do so.
It’s always a good idea to make a note of any medication you are taking and tell us about them when you attend your first appointment, alternatively bring a copy of your repeat prescription along. Please continue to keep your dentist informed of any changes to regular medication and /or medical conditions.
If you are unsure if you pay for NHS treatment, feel free to call the NHS customer enquiry line on 0300 330 1343. They are open Monday to Friday 8am-6pm and Saturday 9am-3pm
If you wish to cancel your appointment, we require 24 hours notice to enable us to make alternative arrangements (perhaps offering your appointment to somebody in pain).
